Non PSIA-AASI Credit Request

NON PSIA-AASI CREDIT REQUEST

In order to receive credit for a non-PSIA-AASI event, we require this request form be submitted for approval at least 30 days in advance.

The request must provide us with details of what the event will entail and its duration. Once you complete this form, PSIA-NW will contact you within 2-3 business days with your request status.

I also understand that not every event will be approved and that I may only use this form one time during a season and may only use this as credit once every three seasons.*
