Member School Clinic Request Form

Reminder; ALL participants must register online to participate in the clinic. Anyone needing education credit can choose that when completing their registration; cost is $50. An email with the link will be sent to you once the clinic is scheduled (pending clinician availability). Registration will stay open roughly 5 days before clinic date (depends on the day of the week), and group size is a maximum of 8.

No registrations will be accepted the day of.

Each education clinic costs $500 and an invoice will be sent to you after the clinic(s); i.e. one clinician/one day = $500. The education clinic fee will be paid in one payment to PSIA-NW via check or can be paid online in the school account via credit card.

CANCELLATION POLICY: CLINICS MAY BE CANCELLED UP TO FOUR WEEKS PRIOR TO THE DATE OF THE CLINIC. IF CANCELLED AFTER, THE FULL AMOUNT OF $500 PER CLINIC PER CLINICAN WILL BE DUE.
